The challenge of this treatment lies in the comprehensive follow up of the device. As the number of implanted devices increases so does the burden of follow up. This is compounded by increasing data provided by devices and increasing sophistication in programming therapy and detection algorithms. There are some general guidelines on pacemaker follow up provided by national organisations, but very little provided in the way of detail. 2 This is reflected in the immense variation in the manner of pacemaker follow up both nationally and internationally. Like most medical interventions pacemaker follow up has to be tailored to the individual. The fundamental principles of pacemaker follow up are listed in table 1.

The impact of beat-to-beat variability in optimising the acute hemodynamic response in cardiac resynchronisation therapy

BACKGROUND Acute indicators of response to cardiac resynchronisation therapy (CRT) are critical for developing lead optimisation algorithms and evaluating novel multi-polar, multi-lead and endocardial pacing protocols. Accounting for beat-to-beat variability in measures of acute haemodynamic response (AHR) may help clinicians understand the link between acute measurements of cardiac function an...

Loss of continuous biventricular pacing in cardiac resynchronization therapy patients: incidence, causes, and outcomes.

INTRODUCTION AND OBJECTIVES In recent years, implantation of cardiac resynchronization therapy devices has significantly increased. The benefits of this therapy are directly related to the maintenance of continuous biventricular pacing. This study analyzed the incidence, causes, and outcomes of loss of continuous biventricular pacing, and the approach adopted. METHODS We analyzed the clinical...

NASPE training requirements for cardiac implantable electronic devices: selection, implantation, and follow-up.

Cardiac implantable electronic devices (CIEDs) have become frequently used therapeutic modalities. CIEDs include pacemakers, implantable cardioverter defibrillators (ICDs), devices for heart failure management (cardiac resynchronization), and implantable hemodynamic monitors. Selection of the appropriate CIED, its implantation, and follow-up care require expertise to achieve optimal therapeutic...

Beneficial effects of right ventricular non-apical vs. apical pacing: a systematic review and meta-analysis of randomized-controlled trials.

AIMS Previous studies have suggested that right ventricular apical (RVA) pacing may have deleterious effects on left ventricular function. Whether right ventricular non-apical (RVNA) pacing offers a better alternative to RVA pacing is unclear. We aimed to conduct a systematic review and meta-analysis of randomized-controlled trials (RCTs) in order to compare the mid- and long-term effects of RV...

Selective-site pacing in paediatric patients: a new application of the Select Secure system.

AIMS The aim of this study was to evaluate the feasibility and reliability of selective-site pacing by means of a new lead system in a paediatric population. This lead system is composed of a 4.1 Fr, active-fixation lead and a steerable catheter that allows easy positioning in selective sites. METHODS AND RESULTS Thirty young patients (mean age 9.0 +/- 4.5 years, range 2-16 years) received a ...
